Required Forms
Parents MUST have a current copy of all applicable forms on record with the school. You can either turn in hard copies to the front desk or email jpeg or pdf copies to registration@olmcboonton.org.

The individual textbook request must be submitted hard copy with original signature. No emailed or scanned copies.
